Enrollment Procedures for April 2021

 Congratulations and welcome to the Graduate School of Environmental Studies (GSES)!
We are pleased to inform you of the enrollment procedure for new students. You must submit the required documents by March 19,2021.
* If you wish to decline admission, please fill out the form “Declining Offer of Admission” and send it back to us as soon as possible.

Date: Documents must arrive at Nagoya University by March 19, 2021 (Friday), 2021.
(We will accept your documents only by mail.)
Otherwise your enrollment may be rejected.

Photo for Student ID Card

Write your name, Furigana, and the serial number on the back of the photo, and attach it to the card.
*Size 3.5 cm long x 3.0cm wide
*In your photo, you must be facing forward and looking straight at the camera in front of a plain white or off-white background, not have a head covering (Unless it's because of religious reasons), and taken in the last 3 months.
※This information on the card will be printed on your student ID.

Write your name, department, date and sign on the form.

Documents regarding Tuition Fee Payments(Request Form for Automatic Withdrawals)

Read the enclosed documents 'Regarding Enrollment and Tuition Fee Payment' and 'Tuition Fee Payment Procedure during the Enrollment Period.'
*All students are required to submit this document whether you are applying for the Tuition Fee Exemption or not.
If you'd llike to open a bank account with Mitsubishi UFJ Bank to set up for a direct debit, read the enclosed "To open an account with Mitsubishi UFJ Bamk" thoroughly.郵送

Application Form for Student Insurance PAS and LSR

This insurance is mandotory. Please refer to the enclosed documents for details.
Please pay the insurance premium using the Insurance Premium Payment Form at a post office.
Attach a receipt to an application form and complete the rest of the form.

Write your name, department, date and sign on the form. Please read “Page 2”.

Fill out with your residential address in Japan and other required information.
Attach your photo on the form.
(Photo data can be pasted.)

Sign your name on both forms,
'For Submission' and 'Duplicate'.
(Keep the 'Duplicate' for your own record.)
Read the enclosed documents 'Information about Xunlei(迅雷)'.

Certificate of Graduation

Take the certificate out from the envelope and write your entrance examination number in the upper right corner of the certificate.
*Your transcript should indicate all the credits you have earned.

Only those who have not fully completed the previous course when taking the entrance examination.
*Master's course students graduated from GSES are not required to submit this document.

The payment receipt of GAKUBAI (CO-OP)

This insurance is also mandatory. Please refer to the enclosed pamphlet "UNIV. CO-OP's KYOSAI" pp.5-6.
Please pay a insurance premium (※) at the NU Co-op Office 2F of the North Cafeteria (Campus Map B2-8) on Higashiyama Campus, and submit a receipt at the time of enrollment.
※CO-OP Membership Fee (¥4,000)+Insurance (¥1,800/1 year) = Total ¥5,800

To apply for the tuition fee exemption, please submit all required documents listed below by post.
Due to the Novel Coronavirus situation, we only accept postal submission this year.
Sending in by email or handing in directly to the Student Affairs Office would not be allowed, and it will be strictly enforced. If you are not able to make the dead line, please email us for the consultation beforehand.
